A Social or Human Services Specialist identifies and arranges community services for clients. Can involve coordination of services for people with disabilities or those in recovery. They may also coordinate housing or job training assistance for immigrants, veterans, former inmates or families in crisis and can be involved in advocating for client needs.

JOB SUMMARY
Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients. RESPONSIBILITIESReasonable accommodation may be made for individuals with disabilities to perform essential job functions. Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management. Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required. Facilitate placement including pre-placement visits, and transportation as needed. Shift coverage including part of the on-call team. Provide field-based services as required by program and client need. Maintain confidentiality of clients care. Participate in scheduled program meetings. Other relevant duties as assigned. Build and maintain partnerships with community and housing agencies. Track and monitor HMIS, AppFolio, and other program systems.
QUALIFICATIONS AA
degree in social service or related field or High School Diploma and 2 years experience. Valid CA drivers license and current auto insurance per contract requirements required. Experience providing mental health and/or substance use disorder services preferred. Cultural competency understanding required. Bilingual preferred. Knowledge of Microsoft Office and EHR software preferred.
