Fresno County Working Hours/Shift:
M-F 8AM-5PM.

SBHG:
Education High School completion required; Associate's degree in a Social Services field preferred Experience Associates degree requires two (2) years of experience performing non-professional, consumer directed services in behavioral health, healthcare, or social services. High School completion requires an additional two (2) years of experience in a social service field.
Licensure & Compliance:
A valid California Driver's License and eligibility to meet agency driving criteria. How you will make a difference: This role assesses clients' housing, education, employment, and resource needs, develops individualized intervention plans, and connects clients with appropriate community services. The position collaborates with treatment teams, maintains community partnerships and resource networks, coordinates transportation and support services, participates in outreach efforts, and ensures accurate and timely documentation in the Electronic Health Record (EHR) system.

Serving adults, 18 plus
Facility & Setting:
Outpatient community-based services and support. Delivers client services throughout Fresno County within designated county limits. May be required to transport clients/client family members utilizing personal vehicle.

Medication management, Housing, Education, and Vocational Support services. Role specific
