Job Description
COMMUNITY SERVICES SPECIALIST
- Alameda OP- 40hrs/week
- Momentum for Health - 3.
- Work schedule is Monday - Friday 8:30am - 5:00pm. BBilingual Spanish speaking candidates encouraged to apply. Bi-Lingual Pay - One hundred thirty-five dollars per paycheck for threshold languages to serve our monolingual clients.
- Pay rates for Union positions are set in the Collective Bargaining Agreement with SEIU Local 521.

a standard vision plan and an enhanced vision planPRINCIPAL DUTIES AND RESPONSIBILITIES
Provide mental health rehabilitation and case management services for a varying caseload, coupled with involvement of their families, which assist and support clients in all areas of daily living and facilitate integration into the community. Duties may include any or all of the following:- Facilitate one-on-one and groups through rehabilitation counseling processes that help develop and strengthen life, coping and independent living skills; develop curriculums.
- Develop and implement individual service and treatment plans; Perform clinical and administrative admissions and discharge paperwork and procedures.
OTHER DUTIES AND RESPONSIBILITIES
Provide appropriate consultation and back-up to support other staff in responding to client crises.- May provide on-call floor coverage, as needed. Provide on-call coverage for non-standard work hours, as needed. Interface with staff, clients, and visitors, as conditions require and in an unobtrusive manner.
- Ensure compliance with applicable health and safety or other regulatory requirements.
- Ensure work area is clean and well kept.
