Social Services Coordinator Sunny Rose Senior Living & Wildomar Senior Assisted Living | Menifee & Wildomar, CA Full-Time | $25-$28 per hour Multi-Community Position Make a Difference for Residents and Families Every Day Sunny Rose Senior Living and Wildomar Senior Assisted Living are seeking a compassionate, organized, and resourceful Social Services Coordinator to support residents and families across both senior living communities. This is a shared position. The Social Services Coordinator will split their work schedule between Sunny Rose Senior Living in Menifee and Wildomar Senior Assisted Living in Wildomar. Candidates must be comfortable working at and traveling between both communities as scheduled. This position is ideal for someone who enjoys helping older adults and their families navigate resources, services, benefits, and changing needs. You will work closely with residents, families, community leadership, healthcare providers, and outside agencies to help connect residents with appropriate support and resources. What You'll Do Support residents and families at both Sunny Rose Senior Living and Wildomar Senior Assisted Living . Build positive, professional relationships with residents and their families. Assist residents and families in identifying social, emotional, and practical needs. Connect residents with appropriate community resources, healthcare services, transportation, benefits, government programs, and other support services. Serve as a resource and advocate for residents while respecting their individual choices, dignity, privacy, and independence. Support new residents and families during the transition into senior living. Maintain regular communication with residents, families, community leadership, and outside service providers. Coordinate referrals and follow up on services and resources. Assist residents and families in navigating changes in needs or circumstances. Participate in resident meetings and interdisciplinary team discussions as appropriate. Maintain accurate and timely documentation related to resident services, referrals, resources, and follow-up. Develop and maintain relationships with local senior service organizations, healthcare providers, government programs, and community resources. Maintain an up-to-date directory of resources available to residents and families. Provide information and education to residents and families regarding available programs and services. Assist with resident and family concerns and coordinate appropriate follow-up with community leadership. Maintain resident confidentiality and comply with applicable community policies and regulatory requirements. Collaborate with departments at both communities to promote a positive resident experience. Perform other related duties as assigned. What We're Looking For Experience in social services, resident services, senior living, assisted living, healthcare, case management, human services, or a related field preferred. Experience working with older adults and families strongly preferred. Degree in Social Work, Human Services, Gerontology, Psychology, Sociology, or a related field preferred, or an equivalent combination of relevant education and experience. Knowledge of community resources and programs available to older adults is a plus. Excellent communication and interpersonal skills. Strong organizational, documentation, and follow-through skills. Ability to manage responsibilities across two senior living communities. Ability and willingness to travel between Menifee and Wildomar as required by the position. Compassionate and professional approach when working with residents and families. Ability to maintain appropriate professional boundaries and confidentiality.
