Job Description

POSITION SUMMARY
The Residential Service Support Specialist works as a member of an interdisciplinary team. The mission of the Residential Service Support Specialist is to provide support to consumers living in, transitioning to, and being admitted to the Program. The purpose is to assist persons served by providing services and supports necessary to assist the recipient in achieving and maintaining rehabilitative, sobriety, and recovery goals. This is intended to meet the mental health or substance abuse needs of adults who have significant functional impairments that seriously interfere with or impede their roles or functioning in family, school, or community. Tasks include initial engagement of consumer to the group home after admission, participation on all assessment and treatment/recovery plan meetings, bi-weekly meetings, program staff trainings, and satisfaction surveys. The Peer Specialist consistently exercises discretion and sound judgment in the performance of the duties of the position.
ESSENTIAL FUNCTIONS OF THE JOB
Provides timely and accurate record keeping and reports as required. Provide written/electronic documentation of formal and informal ongoing assessments of client's progress or barriers to experiencing progress on their goals. Documents contacts and services that relate to the treatment plan. Provides ongoing education to client and family members regarding symptom management and other life skills through modeling, supportive counseling, referral or educational resource materials. Assist clients to directly navigate through public and private systems to obtain needed services. Assists client's in effectively expressing their needs, concerns and desires in working with the agency providing mental health and substance abuse services.
