Baptist Health is seeking a Bachelor's Level Social Services Specialist to join our team. This is a Full-Time Days shift opportunity. The schedule with be M-F 8-hour shifts with no weekends required. The Social Services Specialist will help provide supportive case management, patient and family education, and community resource coordination to improve psychosocial well-being, support treatment adherence, and help patients navigate healthcare needs under physician orders. As a Social Services Specialist, you will be responsible for: Provide medical social services to patients in the home with Physician's Orders Provide rehabilitative and supportive casework to assist patients and their families in adjusting to their health situation Responsible for assisting patients and families to understand, accept, and follow medical recommendations as well as identifying appropriate community resources that are available for the patient Experience Requirements 1-2 years Behavioral Health experience 1-2 years Case Management experience Social Work experience Education and Credential Requirements Bachelors of Science (BS) If you are interested in this Full-Time Days shift opportunity, please apply now.
