Develop a housing procurement and housing retention, financial, and self-sufficiency case management plan with clients. Assist clients in locating and securing housing of their choice. Serve as an ongoing liaison between property managers and participants as well as between participants and neighbors. Apply knowledge of residential lease contracts to educate clients of their rights and responsibilities. Provide information and referral assistance regarding available support from appropriate social service agencies and/or community programs. Assist clients in development of a strength-based/solution-focused individualized goal and action plan that promotes permanent housing and self-sufficiency; develop an effective, timely referral network in order to ensure ongoing direction and support as needed. Assist in development of and encourage adherence to a personal budget through pro-active housing and budget counseling sessions and appropriate referrals. Provide pro-active follow-up home visits to ensure stability and further progress towards self-sufficiency; this includes support, advocacy, reducing isolation, listening, problem solving and identification of resources to assist with reintegration of participants in the community. Apply knowledge of residential lease contracts to educate clients of their rights and responsibilities. Stay current with Georgia Department of Community Affairs Rapid Re-Housing and HOPWA regulations and forms. Maintain accurate daily log records, monthly outcome reports, including

data, and files for each client. Collect and report program data, including but not limited to Homeless Management Information System (HMIS) database reporting and funders' required data.

Work Experience Requirements:

• Two or more years experience in social services field and/or case management or relevant higher education
• Demonstrated experience conducting interviews to gather accurate and relevant information
• Knowledge of homelessness, mental health, HIV/AIDS and assistive services is a plus

Microsoft Word and Excel Housing Coordinator Tasks:

• Foster positive and collaborative relationships with clients, volunteers and staff
• Conduct client intake assessments, develop individualized case plans, and provide ongoing case management support for vulnerable populations
• Communicate professionally and effectively with members of the public
• Assist clients in obtaining suitable affordable housing
• Maintain confidentiality and handle sensitive information with discretion and professionalism
• Collaborate with clients to identify challenges and develop effective solutions
• Maintain accurate records for annual progress reports including client case files and utilizing client databases
• Support clients in maintaining stable housing through crisis intervention, problem solving, and financial planning/assistance
• Build relationships with area property owners and service providers
• Collaborate with social service agencies, mental health providers, addiction specialists, and community organizations to coordinate comprehensive support services
