Community Liaison Job Description:
MultiCare Home Health and Personal Care Services is seeking a Community Liaison to join our team. The Community Liaison will be responsible for the following duties and responsibilities: Communicate with the community about MultiCare Home Health and Personal Care Services services, activities, and programs. Maintain relationships with existing members of the community. Attend weekly staff / marketing meetings. Communicate referral & patient discharge information to intake team. Assist with marketing efforts within the community. Build strong working relationships with community partners, clinical staff and support staff. Assist in developing and implementing marketing plans within the community. Participate in outreach events, such as community fairs, and other opportunities to engage the community.
Qualifications:
The ideal candidate will possess a positive attitude, strong work ethic, a high school diploma or equivalent, and be able to work independently or as part of a team. Candidates must also possess good interpersonal skills and be able to communicate effectively both verbally and in writing. The ability to work independently as well as collaboratively is required for this position. The successful candidate must possess excellent customer service skills and be able to maintain client confidentiality.
