Genoa, IL Job Details Full-time $19 - $29 an hour 2 days ago Qualifications Driver's License Bachelor's degree Computer skills Home visits (communication methods) Full Job Description Position Summary Responsible for developing and implementing each Individual's Personal Plan. Serves as Case Manager and advocate, coordinating programmatic, behavioral, and social supports while maintaining communication with families, guardians, and interdisciplinary teams. Ensures all Individuals receive care in an abuse/neglect-free environment and that all agency policies, DHS/DDD Rules, and timelines are followed.Reports To:
Clinical Director Key Duties & Responsibilities Assist in the Discovery Process and write Implementation Strategies for each Individual's Personal Plan outcomes. Coordinate required signatures, timelines, and documentation per DHS regulations. Ensure Individual participation in planning and goal setting. Complete all required assessments and maintain accurate, timely progress notes. Train staff (RM/DSP) on each Individual's outcomes and behavior support plans. Monitor health concerns, behavior plans, and follow-up actions. Maintain communication with guardians, families, and interdisciplinary teams. Review daily logs and records for accuracy, compliance, and program improvement. Visit each home monthly and maintain strong working relationships with staff and management. Participate in CST/IDT meetings and support transitions, medical appointments, and community events. Report incidents immediately and ensure knowledge of emergency procedures. Stay current on DHS/DDD regulations, agency policies, and required trainings. Serve as agency liaison for hospitalizations and community activities. Support best practices and continuous quality improvement across agency programs. Perform other duties as assigned by the Clinical or Executive Director. Qualifications Bachelor's degree in Social Services or related field Minimum 1 year of experience with Individuals with Intellectual Disabilities Certified DSP or ability to obtain certification Valid Illinois driver's license, clean record, and ability to lift 50 lbs Strong organizational, communication, and computer skills Work Environment Primarily in CILA homes, offices, and community sites.
