PRN / Part-Time (1-2 days per week) About Us Consistent Care Personal Service Agency provides compassionate adult day services, transportation, and home care support to seniors and adults needing assistance in the community. Position Summary We are seeking a motivated Community Liaison to help grow referrals for our Adult Day Program and Home Care services. This position is ideal for someone with experience in healthcare marketing, home health, hospice, senior living, case management, or community outreach. Most outreach will be conducted through phone calls, email, and relationship-building with referral sources. Responsibilities Develop and maintain referral relationships Contact case managers, social workers, healthcare providers, and community organizations Follow up on referral opportunities Track outreach activities and referrals Assist with scheduling tours and intake appointments Represent the organization professionally in the community Qualifications Healthcare, senior services, home care, hospice, or community outreach experience preferred Strong communication and relationship-building skills Organized and self-motivated Comfortable working independently Experience with Microsoft Office and Google Workspace preferred Preferred Background Home Health Marketer Hospice Liaison Community Relations Representative Senior Living Sales Healthcare Outreach Coordinator Compensation $20-$25/hour based on experience Flexible schedule Opportunity for performance incentives To Apply Please submit your resume and a brief description of your healthcare outreach or referral development experience.
