Pay Rate $25 per hour + benefits! Centurion is proud to be the provider of comprehensive healthcare services Indiana Department of Correction. We are seeking a Special Needs Liaison for needs at correctional facilities throughout the state of Indiana. •This position requires extensive, frequent statewide travel.•The Special Needs Liaison develops and implements transitional healthcare plans for specific releases with complex medical needs. This position requires maintaining detailed data and ongoing spreadsheets with specifics as well as coordinating and working closely with IDOC (Indiana Department of Correction) staff, Centurion transitional healthcare facilitators and liaisons and other state or local agencies to ensure that necessary pre-discharge planning and preparation is accomplished for individuals leaving the prison environment and entering the community. Bachelor's degree in human services, social services, or criminal justice field Two years of experience working in a correctional setting Experience with/knowledge of community and state programs and services Two years of experience in re-entry services or release planning preferred Must maintain current tuberculosis screening documentation and active CPR and Automated External Defibrillator (AED) use certification Must be able to pass background investigation and obtain agency security clearance where applicable
Available Shift:
