Job Description

PACT Housing Specialist Schedule:
Mon-Fri 8:00am-4:00pm
Salary:
$54,202.00 annually
Sign On:
$5K Help individuals with mental health challenges find and maintain safe, stable housing while making a meaningful impact in your community. We are seeking a compassionate and motivated PACT Housing Specialist to join our multidisciplinary Program of Assertive Community Treatment (PACT) team. In this role, you will support adults living with serious mental illness by helping them secure housing, build independent living skills, connect with community resources, and achieve their recovery goals. Responsibilities Assist individuals in finding and maintaining safe, affordable housing. Develop relationships with housing authorities, landlords, and community partners. Provide case management, advocacy, and individualized support in both office and community settings. Teach independent living skills, including budgeting, transportation, and daily living activities. Participate in treatment planning as part of a multidisciplinary team. Coordinate care with families, healthcare providers, and community agencies. Complete timely documentation and maintain accurate records. Participate in an on-call rotation for crisis support. Qualifications High school diploma or equivalent required; Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred. 2-3 years of experience supporting adults with serious mental illness, preferably with housing or case management responsibilities. Knowledge of housing authorities and federal/state housing programs is preferred. Strong communication, problem-solving, and advocacy skills. Valid driver's license for 6+ months, reliable transportation, and ability to travel within the community. Why Join Us?
