Home Health Marketer / Community Liaison - Northwest Michigan About Us Northern Homecare is a family-owned home health agency dedicated to providing compassionate, high-quality care to patients throughout Northwest Michigan. As a locally rooted organization, we take pride in building strong relationships with the communities and healthcare partners we serve. Position Summary Northern Homecare is seeking a full-time Home Health Marketer to build and maintain referral relationships across our Northwest Michigan territory, spanning from Frankfort to Muskegon. This role is key to growing our patient census by connecting physicians, hospitals, skilled nursing facilities, senior living communities, and other referral sources throughout the region. Key Responsibilities Develop and maintain strong referral relationships with physicians, hospitals, discharge planners, skilled nursing facilities, assisted living communities, and other healthcare partners Conduct regular in-person visits to referral sources throughout the Frankfort-to-Muskegon territory Represent Northern Homecare's services, mission, and values to the community Identify and pursue new referral opportunities to support census growth Track outreach activity and referral trends; report on marketing efforts and results Collaborate with clinical and intake teams to ensure smooth referral-to-admission transitions Attend community events, health fairs, and networking functions to promote agency visibility Qualifications Prior experience in healthcare marketing, sales, or business development preferred (home health or hospice experience a plus) Strong interpersonal, relationship-building, and communication skills Self-motivated with the ability to manage a large geographic territory independently Valid driver's license and reliable transportation required Proficiency with basic computer/CRM tools for tracking outreach What We Offer Full-time position, $20-$21/hour Mileage reimbursement for all territory travel Family-owned company culture with a genuine commitment to patients and staff Opportunity to make a meaningful impact on home health access across
