A Direct Service Professional provides direct services for consumers in a community, home-based and/or office-based setting; provides skills training and development, community living supports, out-of-home habilitation and prevocational services, health and safety, and transportation services; completes consumer progress notes for face-to-face meetings and significant phone or other events; and follows up with Person-Centered Planning goals and objectives through appropriate documentation. A Direct Service Professional coordinates services for consumers appropriate to their wants and needs and the Person-Centered Plan; links consumer to agency and community resources for which they are eligible; and advocates for individuals in areas of health and safety, placement, financial, skill-building, social, recreational and family relationships. A Direct Service Professional completes documentation required by the agency, Michigan Department of Health and Human Services, Commission on Accreditation on Rehabilitation Facilities and other regulatory bodies as required for clinical staff. A Direct Service Professional performs other related duties as assigned. A. Required Experience and Training Be a high school graduate or have a certificate of completion of the General Educational Development (GED) test. Have one (1) year of experience in a human service role. Please note, internships count as work experience if they are documented in the Work Experience section of the employment application. Possess a valid driver's license and the ability to obtain a Michigan Driver License within 30 days of employment. Must have reliable transportation that may have to be used to carry out job duties of this classification. B. Preferred but not Required Bilingual candidates highly desired.
