As a Home Health Community Liaison , you'll connect older adults and their families with the services they need while building relationships throughout the St. Louis community. This position focuses on sales, community outreach, relationship development, and referral growth for EverTrue Anywhere Care and the broader EverTrue continuum of care. Key areas of focus include Anywhere Care, hospice, palliative care, private duty/home help, and home health . This position includes a structured sales incentive opportunity tied to referral growth and business development results. What You'll Do Build and maintain relationships with hospitals, physicians, discharge planners, senior living communities, healthcare providers, and other referral sources. Market EverTrue services, develop new referral partnerships, and generate appropriate referrals, enrollments, and admissions. Conduct community outreach, networking, educational presentations, and follow-up with referral partners. Promote EverTrue as a provider of choice for hospice, palliative care, home help/private duty, and home health services. Connect clients and referral partners with the appropriate services across the EverTrue continuum of care. Track referral activity and pipeline progress while participating in sales meetings and quarterly goal planning. Maintain knowledge of Medicare, Medicaid, managed care, insurance contracts, and applicable healthcare services. What We're Looking For Bachelor's degree required Prior sales, marketing, business development, or healthcare liaison experience preferred Knowledge of Medicare, Medicaid, home health, hospice, or related healthcare services Strong communication, presentation, networking, and relationship-building skills Self-motivated and comfortable working independently Comfortable in a clinical environment and familiar with medical terminology Strong computer/database skills Ability to travel daily throughout the St. Louis metro area Why Join EverTrue?
