Job Description
Overview Vital Homecare is seeking an energetic, relationship-driven Community Liaison/Home Care Marketer to grow our presence and generate qualified home-care referrals in the Asheville market. The Community Liaison will represent Vital Homecare throughout the local healthcare community, build lasting relationships with referral partners, identify individuals who may benefit from personal care services, and work closely with our admissions and operations teams to support a smooth transition from referral to start of care. This is a field-based business-development position for someone who is comfortable meeting referral sources in person, following up consistently, tracking results, and being accountable for measurable census growth.
Primary Responsibilities:
- Develop and maintain relationships with hospitals, rehabilitation facilities, skilled nursing facilities, assisted living communities, physician offices, social workers, case managers and discharge planners.
- Build referral relationships with Medicaid managed-care organizations, care-management organizations, CAP/DA case managers, Veterans Affairs contacts and community-based organizations.
- Educate referral partners about Vital Homecare's personal care and in-home support services.
- Identify new referral opportunities and maintain consistent follow-up with prospective referral sources.
- Conduct in-person visits, presentations, networking meetings and community outreach activities.
- Respond promptly to questions from referral partners and communicate service-area and staffing availability.
- Gather the information needed to determine whether prospective clients may qualify for services.
- Coordinate closely with the Admissions Coordinator to ensure referrals are contacted, screened and processed promptly.
- Follow referrals from initial receipt through authorization, admission and start of care.
- Maintain accurate documentation of visits, contacts, referrals and outcomes in the company's tracking system.
- Provide weekly reports covering outreach activity, new referral relationships, referrals received, admissions and barriers to conversion.
- Attend community events, healthcare networking meetings, provider meetings and industry events.
- Monitor competitor activity and identify unmet home-care needs within the territory.
- Maintain professional, ethical and compliant marketing practices at all times.
- Protect patient confidentiality and comply with HIPAA and all applicable company policies. Performance Expectations Following the initial ramp-up period, the Community Liaison will generally be expected to:
- Complete at least 15-20 meaningful referral-source visits or meetings each week.
- Build and maintain an organized territory list of active and prospective referral sources.
- Establish at least 10 new qualified referral-source relationships each month.
- Generate a consistent pipeline of qualified referrals that can convert into staffed admissions.
- Meet agreed-upon monthly referral, admission and census-growth goals.
- Document outreach activity and referral outcomes accurately and on time.
- Maintain strong communication and service standards with both referral partners and internal departments. Performance will be evaluated based on a balanced combination of outreach activity, qualified referrals, completed admissions, census growth, referral quality, retention and professional conduct. Qualifications
- At least two years of experience in healthcare marketing, business development, community outreach or outside sales preferred.
- Home care, personal care, home health, hospice, Medicaid, senior-care or healthcare referral experience strongly preferred.
- Existing relationships within the Wilmington or Asheville healthcare community are highly desirable.
- Demonstrated ability to build professional relationships and generate new business.
- Strong communication, presentation, organization and follow-up skills.
- Comfortable working independently and spending most of the workweek in the field.
- Ability to track activity and results using spreadsheets, CRM software or similar systems.
- Knowledge of Medicaid personal care services, managed care, CAP/DA or VA community-care programs is a plus.
- High school diploma or equivalent required; associate's or bachelor's degree preferred.
- Valid driver's license, reliable vehicle and ability to travel throughout the assigned territory required.
- Occasional early-morning, evening or community-event availability may be necessary. What We Offer
