A Social or Human Services Specialist identifies and arranges community services for clients. Can involve coordination of services for people with disabilities or those in recovery. They may also coordinate housing or job training assistance for immigrants, veterans, former inmates or families in crisis and can be involved in advocating for client needs.
Age Well Center is seeking a compassionate and dependable Outreach Specialist to help older adults and their families access services and resources within our community. The Outreach Specialist works directly with adults age 60 and older through home visits, office visits, and telephone contact. This position helps identify each client's needs, connects them with appropriate programs and services, assists with applications, and follows up to ensure they receive the support they need. This is a meaningful position for someone who enjoys working with older adults, building relationships, and helping people navigate challenging situations.
SCHEDULE
Part-time, approximately 25 hours per week Monday through Friday Current schedule is 10:00 a.m.-3:00 p.m.; however, hours can be flexible, such as 9:00 a.m.-2:00 p.m. No evenings or weekends Age Well Center is closed on recognized holidays
PRIMARY RESPONSIBILITIES
Meet with older adults and their families to identify needs and develop client-centered plans Provide information and referrals for community services, including food assistance, transportation, housing, home care, healthcare, financial assistance, and other senior services Conduct home visits throughout Burleigh County Complete annual assessments for Home Delivered Meals clients Assist clients with Medicaid applications and other public-benefit programs Provide information about publicly funded long-term care programs and assist clients through eligibility and enrollment processes Maintain accurate and confidential client records and documentation Follow up with clients to determine outcomes and provide additional assistance when needed Develop and maintain positive relationships with community agencies and local service providers Work closely with the Home Delivered Meals team to support the safety and well-being of clients Assist with meal delivery when volunteer coverage is unavailable Deliver frozen meals on Fridays based on current client needs Cross-train in Home Delivered Meals duties to assist with department coverage when needed Attend applicable workshops, seminars, and training sessions Fulfill mandated-reporting responsibilities related to suspected elder abuse, neglect, or self-neglect Perform other duties as assigned
QUALIFICATIONS
At least two years of experience working with older adults in healthcare, human services, social services, or another service-oriented setting A degree in social work, human services, or a related field is preferred but not required Experience working with individuals living with dementia or memory loss is strongly preferred Familiarity with community programs, public benefits, and senior-service resources in the Bismarck-Burleigh County area is preferred Strong communication, organization, documentation, and problem-solving skills Compassionate and patient, with the ability to build trust while maintaining professional boundaries Ability to handle confidential information appropriately Valid driver's license and ability to travel throughout Burleigh County
