Hospice Community Liaison LilyCare Hospice is a community-based hospice serving southern New Hampshire out of Bedford. We care for patients where they live — private homes, assisted living, and skilled nursing facilities. We are a growing, nurse-driven organization, not a national chain. Our Hospice Community Liaison owns the territory outright and reports directly to leadership. We are hiring a Hospice Community Liaison to build our southern New Hampshire referral base. Benefits Medical, dental, and vision insurance 401(k) Paid time off and paid holidays Mileage reimbursement for all territory travel Duties and Responsibilities Build and maintain referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices across the assigned territory Educate referral sources on hospice eligibility, the Medicare hospice benefit, and the LilyCare model of care Develop and execute a quarterly territory plan with defined target accounts and measurable referral goals Plan and deliver in-services and community education events, managing the associated marketing budget Compensation and Schedule The Hospice Community Liaison role pays $85,000-$100,000 per year plus performance bonus. The pay range listed represents a general guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate's relevant experience and qualifications.
