$25 per hour Position Summary We are seeking a compassionate, motivated, and relationship-focused Community Liaison to represent our Home Health, Hospice, and Wound Care services within the community. The Community Liaison will develop and maintain strong relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, senior living organizations, case managers, discharge planners, social workers, and other healthcare professionals.

The ideal candidate will understand the healthcare referral process and be passionate about connecting patients and families with high-quality, compassionate care in the comfort of their home or residential setting. Key ResponsibilitiesCommunity & Referral Development Develop and maintain professional relationships with healthcare providers and community organizations. Identify and establish new referral relationships for Home Health, Hospice, and Wound Care services. Conduct regular visits to physician offices, hospitals, skilled nursing facilities, assisted living communities, and other referral sources. Educate referral partners about available services, patient eligibility, and the referral process. Respond promptly to referral opportunities and coordinate communication between referral sources and the clinical team. Participate in community events, healthcare networking events, senior fairs, educational programs, and professional meetings. Maintain consistent communication with referral partners to strengthen long-term relationships. Patient & Family Education Provide general information about Home Health, Hospice, and Wound Care services to patients, families, caregivers, and healthcare professionals. Help families understand available services and how to access appropriate care. Promote awareness of hospice philosophy, advance care planning, and supportive end-of-life services when appropriate. Educate communities about the importance of specialized wound care and prevention of wound-related complications. Business Development Meet established referral-development and community outreach goals. Maintain a structured schedule of community and referral-source visits. Track referral activity, outreach efforts, and relationship development. Identify opportunities to expand services and increase community awareness. Collaborate with leadership and clinical teams to address referral-source needs and improve service delivery. Monitor market trends and identify opportunities for community partnerships. Communication & Collaboration Work closely with clinical, intake, operations, and administrative teams. Communicate referral-source feedback and community needs to leadership. Maintain professionalism, confidentiality, and compliance with all applicable healthcare regulations. Represent the organization positively and professionally at all times.
Qualifications Required:
High school diploma or equivalent; associate or bachelor's degree preferred. Previous experience in healthcare, home health, hospice, wound care, senior care, healthcare marketing, or business development for two (2) years preferred. Strong interpersonal and communication skills. Professional appearance and excellent relationship-building abilities. Ability to work independently and manage a community-based schedule. Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area. Strong organizational and time-management skills.
Preferred Experience:
Home Health or Hospice marketing/business development. Wound care or post-acute healthcare. Hospital discharge planning or referral coordination. Skilled nursing, assisted living, or senior living relationships. Experience working with physicians, case managers, social workers, and healthcare administrators. Pay:
$25.00 per hour
