The Clinton Avenue Apartment Program is seeking a Housing Specialist to join our supportive housing team in Albany, NY. This is a full-time position working Tuesday through Saturday, 8:00 a.m. to 4:30 p.m. , with some scheduling flexibility. As a Housing Specialist , you will provide intensive case management and supportive services to individuals with mental health diagnoses who are transitioning from homelessness or institutional settings into permanent supportive housing. About the Program The Clinton Avenue Apartment Program is an ESSHI (Empire State Supportive Housing Initiative) single-site permanent supportive housing program with 20 apartment units, including 10 Permanent Supportive Housing (PSH) apartments with Rehabilitation Support Services (RSS). Two Housing Specialists work collaboratively to provide individualized support to RSS tenants while also offering resource support to other low-income residents in the building. Responsibilities The Housing Specialist will manage a caseload of 8-10 RSS tenants and will: Provide intensive case management and supportive services. Develop, implement, and monitor individualized service plans. Assist tenants with skill development to promote independent living. Support and facilitate tenant meetings. Coordinate community presentations, workshops, and resource opportunities. Help ensure RSS tenants maintain safe, clean, and healthy apartments. Connect tenants with community resources and advocate on their behalf. Complete required documentation and maintain accurate case records. Participate in the agency's on-call rotation for one week each quarter. Perform other duties as assigned. Qualifications High school diploma or equivalent required. Valid New York State driver's license required. Experience working with individuals with mental health diagnoses, homelessness, or supportive housing is preferred. Strong communication, organizational, and interpersonal skills. Ability to work independently and as part of a collaborative team. Why Join Us?
