Housing Specialist Program:
Healthy Alliance Social Care Network (SCN) Pilot Program Full Time, Non-Exempt 19.25/hr
M-F 9AM-5PM

This position is part of a one-year pilot program scheduled to end 3/31/27. Continued employment is dependent on program success and renewed funding. Position Summary New York State Medicaid has expanded services that address Health Related Social Needs (HRSNs), recognizing that stable housing and access to basic needs are essential to good health. The Housing Specialist plays a supportive and practical role in helping eligible Medicaid members access temporary housing related assistance. Services are short-term, up to 6 months, and are designed to help individuals and families obtain, or maintain safe and stable housing. This position provides screening, eligibility assessment, navigation, coordination, and assistance with various housing supports including rent, rental arrears, utilities, furnishings, and more, along with follow up to close the referral. This is not an emergency or on-call position. Instead, the role focuses on helping a participant find, obtain, or maintain permanent housing or supports after eligibility is established. Clients served by this program are located in St. Lawrence and Franklin Counties. While most communication and follow-up will occur by phone, willingness to travel throughout the service area is essential to the position. Essential Duties 1. Screening, Identification, and Assessment Complete standardized screenings using an online system. Document information clearly and accurately. Follow up with participants in a timely and supportive manner. Help determine eligibility for housing services. Ask required follow-up questions to clarify unmet needs and document responses. Identify other helpful resources for referral, such as SNAP, WIC, TANF, care management, or local housing programs. 2. Social Care Plan Development and Maintenance Develop individualized housing-focused care plans. Update plans as goals are met or services change. Ensure participants understand their options and feel supported. 3. Housing-Related Services Depending on client eligibility, the Housing Specialist may help coordinate: Rental Subsidy or Arrears up to 6 months. Utility Set up or Arrears of one-time assistance. Pre-tenancy services including housing search, applications, landlord communication. Community Transition Supports including one-time startup costs of security deposits, moving expenses, or essential furnishings. Tenancy Sustaining Services including recertification paperwork, or connecting to other providers. Housing Transition and Navigation to help those who are transitioning from institutional settings. Ongoing support including outreach, education, and follow up with the participant to address their needs. This position is not expected to solve housing shortages or provide emergency shelter. The role is to help navigate and access available supports and make eligible payments for approved services. 4. Working with Diverse Populations Participants may include persons with physical, intellectual, or mental health disabilities. Individuals with limited incomes. And, individuals with prior criminal justice involvement. 5. Documentation and Compliance Maintain accurate and timely documentation in electronic systems. Collect required eligibility paperwork before services begin. Adhere to quantity limits of services. Protect member confidentiality and obtain appropriate consents. Participate in reporting of services and quality improvement efforts. 6. All other Necessary Tasks Assigned Requirements Associate's degree in human services or related preferred. All education and work experience will be considered. We value a combination of education, lived experience, and professional experience. Experience supporting individuals with housing or case-management needs is helpful. The preferred candidate will have experience providing support services, case-management like activities, and/or linkages to other agencies. Personal experience with disability a PLUS. Experience with case documentation and electronic data systems. Ability to manage multiple tasks with different deadlines. Excellent documentation and organizational skills. Cultural humility and respectfulness of all served. Many clients will have barriers, including disabilities. The ideal candidate will have experience working with individuals with varied backgrounds in a professional manner. Computer and technology skills required. This includes typing, using Microsoft Office products, scanning and uploading documents, using e-signature software, data entry, using internet browsers, using printer machines, using cell phones, and using calendar software through Outlook. Willingness and ability to travel throughout the service area to promote the housing program and meet with potential participants. Individuals served will be located in St. Lawrence and Franklin Counties. A valid driver's license and vehicle insurance required. Travel requires the use of your own mode of transportation. Benefits & Pay Full time, 40hpw, non-exempt Monday-Friday 9AM-5PM paid lunch and breaks, therefore the work week is 35hpw 19.25 her hour Generous Benefits Package 11 paid holidays 80 paid hours sick time 80 paid hours vacation annually. Years 1-4 are 80hrs per year. Year 5 increases to 120 hours, and every year after that the employee is provided with an additional 8 hours of vacation time (ie: Year 6 = 128, Year 7 = 136, and so forth). 32 paid hours personal time Paid bereavement Paid lunch and breaks Paid Snow Days & Snow Delays (based on local school closure) Paid jury duty leave 403b Retirement plan with an employer 3% contribution after 2 years of employment. Excellent health insurance for the employee! Single coverage at only 60.00 a month premium. MILC provides an HRA that pays 75% of the medical copays/coinsurance costs. Dental and vision costs 5.00 a month for the employee. MILC does not offer Family Health Insurance, but we do offer Family Dental/Vision for an additional cost. Mileage reimbursement at the IRS rate (0.76/mile for 2026) Private office Scheduling flexibility available upon request to accommodate medical/personal appointments All other required benefits such as Worker's Compensation, Paid Family Leave, Unemployment, Short Term Disability.
