For many individuals, recovery begins with having a safe place to sleep, reliable transportation, and access to community support. Talbot Health Services is seeking a Housing Specialist to work directly with individuals receiving substance use and mental health treatment services. This position is responsible for helping clients overcome housing barriers, connect with community resources, and build a foundation for long-term recovery. This role is ideal for a CDCA who enjoys working directly with people, solving problems, building community partnerships, and making a visible impact in clients' lives every day. What You'll Do Assist clients in locating and securing stable housing opportunities. Coordinate referrals to sober living, transitional housing, shelters, and permanent housing programs. Work directly with landlords, housing agencies, and community organizations. Help clients obtain identification, benefits, transportation resources, and other supportive services. Support treatment engagement by addressing barriers that may impact recovery. Maintain documentation and service records in accordance with agency requirements. Participate in multidisciplinary treatment team meetings. Advocate for clients while promoting accountability and self-sufficiency. What We're Looking For Required Active Ohio CDCA certification. Valid driver's license and reliable transportation. Strong communication and relationship-building skills. Ability to work independently and manage multiple priorities. Preferred Experience working with substance use disorders, mental health services, homelessness, or community-based programs. Outpatient Case Manager (CDCA Required) Outpatient Case Manager Talbot Health Services | Toledo, Ohio Be the Connection Between Treatment and Recovery Talbot Health Services is seeking a compassionate and motivated Outpatient Case Manager to support individuals receiving substance use and mental health treatment services. Successful recovery often requires more than counseling alone. Clients may face barriers involving employment, transportation, housing, healthcare, legal obligations, family relationships, and financial stability. As an Outpatient Case Manager, you will help individuals navigate these challenges while supporting their treatment goals and long-term success. This position is ideal for someone who enjoys building relationships, advocating for others, and helping clients create meaningful change in their lives. Key Responsibilities Provide case management services to individuals receiving outpatient treatment. Assist clients in accessing community resources and support services. Coordinate care with counselors, medical providers, probation officers, employers, and other referral partners. Develop individualized service plans based on client needs and goals. Monitor progress and maintain ongoing client engagement. Complete timely and accurate documentation. Support discharge planning and continuity of care efforts. Participate in treatment team meetings and collaborative care planning. Qualifications Required Active Ohio CDCA certification. Valid driver's license and reliable transportation. Strong interpersonal, organizational, and communication skills. Preferred Previous experience in behavioral health, addiction treatment, community mental health, social services, or healthcare.
