This role works closely with the Philadelphia Police Department, Fire Department, diversionary courts, and other city partners to divert individuals from the criminal justice system into social and behavioral health services.
Key Responsibilities:
Conduct outreach and case management, providing support, guidance, and warm handoffs to community members. Build trust with individuals, mentor them in goal-setting, self-advocacy, and life skills. Coordinate care with internal and external providers, including courts and diversion programs. Assist with access to city resources such as housing, medical care, food, transportation, and employment. Participate in supervision, team meetings, and quality improvement initiatives. Maintain professionalism, model accountability, and use de-escalation and situational awareness strategies. Earn $30.11/hour This position is Sunday - Thursday 7:00a - 3:30p.Location:

The ideal candidate will possess the following qualifications:Required:
Bachelor's degree in Social Work, Criminal Justice, Psychology, Counseling, Human Services, or a related field. Knowledge of recovery principles and experience working within a recovery-oriented, person-centered service system. Strong ability to work both independently and collaboratively as part of a multidisciplinary team. Ability to build and maintain professional relationships with city agencies and partners, including the Office of Public Safety, Philadelphia Police Department (PPD), Philadelphia Fire Department, and internal/external stakeholders. Driver's license Preferred 1-3 years of case management experience in a human services setting; experience in drug and alcohol treatment strongly preferred.Benefits
