Job Description

Healthcare Community Liaison Build relationships. Make a difference. Help connect individuals and families with compassionate, high-quality home care. Emerald Grace HomeCare is seeking a motivated, professional, and relationship-driven Healthcare Community Liaison to help grow our presence throughout Northeast Georgia. This position is ideal for someone who enjoys networking, meeting new people, and developing lasting professional relationships within the healthcare community. As the Healthcare Community Liaison, you will serve as the face of Emerald Grace HomeCare by educating healthcare professionals, community organizations, and families about our services while developing referral partnerships that support our continued growth. Responsibilities Develop and maintain strong relationships with hospitals, physician offices, rehabilitation facilities, skilled nursing facilities, hospices, senior living communities, case managers, social workers, veterans' organizations, and other referral sources. Visit healthcare facilities and community organizations to educate professionals about Emerald Grace HomeCare's services. Establish new referral relationships while maintaining existing partnerships. Attend networking events, health fairs, community events, vendor fairs, and other outreach opportunities. Deliver brochures, business cards, and educational materials to referral partners. Follow up with referral sources to maintain strong professional relationships. Identify opportunities to increase community awareness and expand referral partnerships. Track outreach activities, referrals, and business development efforts. Assist with planning educational presentations and community outreach initiatives. Represent Emerald Grace HomeCare with professionalism, integrity, and compassion at all times. Collaborate with leadership to support the agency's growth and outreach goals. Qualifications Excellent communication and interpersonal skills. Strong networking and relationship-building abilities. Professional appearance and positive attitude. Self-motivated with the ability to work independently. Strong organizational and time-management skills. Valid driver's license, reliable transportation, and willingness to travel throughout the service area. Experience in healthcare, home care, hospice, medical sales, community outreach, or business development is strongly preferred but not required. Knowledge of the local healthcare community is a plus. Preferred Skills Comfortable speaking with healthcare professionals, community leaders, and families. Ability to build trust and establish long-term professional relationships. Strong follow-up skills and attention to detail. Proficient with Microsoft Office, email, and basic computer applications. Experience maintaining referral, customer, or CRM databases is preferred.
