Job Summary We are seeking a dedicated and compassionate Housing Navigator specializing in case management to join our dynamic team. In this vital role, you will serve as a guiding resource for individuals experiencing homelessness or housing instability, helping them navigate complex housing assistance programs and connect with stable, affordable housing options. Your energetic approach will empower clients to overcome barriers, access essential services, and achieve long-term housing stability. This position offers an opportunity to make a meaningful difference in underserved communities by providing personalized support rooted in social work principles and crisis intervention strategies. Responsibilities Engage with clients to assess their housing needs, develop individualized action plans, and provide ongoing case management support. Assist clients in understanding and applying for housing assistance programs such as LIHTC (Low-Income Housing Tax Credit), Section 8, and other HUD (Housing and Urban Development) initiatives. Negotiate with landlords and property managers to secure housing placements that meet clients' needs while adhering to Fair Housing regulations and landlord-tenant law. Conduct crisis intervention when necessary, offering immediate support for clients facing mental health challenges, addiction issues, or high-risk situations. Utilize property management tools like OneSite to track client progress, document interactions, and coordinate services efficiently. Provide culturally sensitive communication and support to diverse populations, including individuals from underserved communities and those experiencing homelessness. Collaborate with social services agencies, mental health providers, addiction counselors, and community organizations to coordinate comprehensive care plans for clients. Qualifications Bachelor's degree in Social Work, Psychology, Nursing, or a related field (Master's preferred). Minimum of 2 years of experience in case management, social services, or a related field. In-depth knowledge of local health and social service resources in Contra Costa County. Strong proficiency in care coordination principles, electronic health records, and case management software. Excellent communication, interpersonal, and organizational skills; bilingual abilities are a plus. Valid driver's license and reliable transportation. Why Join Us?
