CST Team Lead Position Summary The Qualified Professional (QP) is a key clinical member of the Community Support Team (CST) at Arbor Care Solutions. This position provides direct, rehabilitative services to adults with mental health and/or substance use diagnoses through community-based, face-to-face interventions. The QP collaborates with a multidisciplinary team to support client recovery, symptom reduction, and successful community integration. This role also includes participation in a rotating 24/7 on-call crisis response schedule, including some weekend availability.
Key Responsibilities:
Direct Clinical and Rehabilitative Services Deliver in-person clinical and rehabilitative services in community settings (homes, shelters, public areas). Provide psychoeducation, skill-building, and supportive counseling aligned with the Person-Centered Plan (PCP). Assist clients in developing independent living, emotional regulation, and coping skills. Support relapse prevention, safety planning, and functional needs assessments. Accurately document all interventions in the electronic health record (EHR) according to Medicaid and agency standards. Crisis Response & On-Call Coverage Participate in a rotating 24/7 on-call crisis schedule, including evenings and weekends as needed. Respond to mental health and substance use crises with in-person interventions. Collaborate with the CST Team Lead to assess safety, stabilize the situation, and initiate appropriate care. Assist with the development and implementation of crisis, WRAP, or psychiatric advance directives. Care Coordination & Case Management Connect clients with resources such as housing, benefits, legal aid, healthcare, and employment. Assist with transitions from hospitals, jails, or residential treatment to the community. Coordinate across service systems and ensure follow-through on referrals. Advocate for clients across multiple life domains to promote community integration. Treatment Planning & Documentation Collaborate in the development, monitoring, and revision of Person-Centered Plans (PCPs). Ensure service delivery is goal-directed, measurable, and clinically appropriate. Maintain accurate and timely documentation in the EHR system. Track and communicate progress and barriers to the CST Team Lead. Team Collaboration Participate in weekly CST meetings, treatment reviews, and clinical staffing. Support APs, PPs, and Peer Support Specialists with coordination and updates. Provide input to promote team-based care and alignment with service goals. Quality Assurance & Compliance Follow all Medicaid guidelines, agency policies, and ethical standards. Participate in chart audits, quality improvement efforts, and ongoing training. Ensure all services align with clinical coverage requirements and licensing board expectations.
Qualifications:
Required Credentials (must meet one of the following): Master's in a human services field + 1 year of full-time post-grad MH/SUD experience Bachelor's in a related human services field + 2 years of post-degree MH/SUD experience Bachelor's in a non-related field + 4 years of post-degree MH/SUD experience Additional Requirements At least one year of experience serving adults with MH/SUD Valid NC driver's license and reliable transportation Strong communication, documentation, and problem-solving skills Willing to work in homes, shelters, and various community settings Must be available for rotating crisis response, including evenings and weekends Training Requirements Complete at least 20 hours of CST-related training within 90 days of hire Participate in continuing education and modality-specific skill development as required Compensation & Benefits Competitive pay based on qualifications and experience Medical, Dental, Vision Insurance Short-Term Disability, Whole Life Insurance Paid Time Off (PTO) and holidays Mileage reimbursement for travel Supervision toward licensure (if applicable) Work Schedule Monday to Friday | 8:00 AM-5:00 PM Full-Time Only Includes rotating on-call crisis coverage and some weekend availability Why Join Arbor Care Solutions?
