Who we need:
We are looking for a qualified, motivated, and experienced Resident Services Associate for a residential community located in [West Hartford, CT]. [Monday thru Friday, 8:30 am to 5:00 pm]. The Resident Services Coordinator (RSC) is responsible for assisting tenants to identify services and benefits that will enable tenants to maintain independence. The RSC works in conjunction with the Property Manager and always maintains a mutually respectful, collaborative relationship. The objective of the RSC position is to work from an empowerment model toward the goal of self-actualization of the residents. The RSC must fulfill many roles to meet this goal including educator, community builder, advocate, investigator, liaison, and service facilitator. What you'll do: Coordinate supportive services for residents, with a focus on helping older adults maintain independence and successfully age in place. Assess residents' psychological, physical, social, and service needs and provide appropriate referrals and follow-up support. Connect residents with community resources and entitlement programs, including transportation, meals, home health services, counseling, financial assistance, and healthcare resources. Build and maintain partnerships with community agencies, healthcare providers, and social service organizations to expand resident support options. Organize educational programs, wellness initiatives, social activities, and community events for residents. Assist residents and families with care planning and transitions to higher levels of care, including nursing home placement when necessary. Support property management by addressing tenant concerns, assisting with conflict resolution, and responding to unusual resident behavior or issues identified during inspections and recertifications. Maintain confidential resident service records, case notes, releases, and documentation in compliance with HUD and company requirements. Prepare monthly and annual reports and maintain accurate documentation for owners, management, and HUD compliance.
