Community Liaison / Home Care & Marketing Representative Employment Status Part-time 20 hours per week Compensation Base pay: $20.00 - $25.00/hour Commission $200 for every new Medicaid client who: Completes admission, Begins receiving services, and Remains active for at least 30 days. Monthly Performance Bonus $500 bonus when 5 or more new Medicaid clients become active during the same calendar month. Responsibilities Market All Hearts Home Care to: Hospitals Rehabilitation facilities Skilled nursing facilities Physician offices Case managers Social workers Senior living communities Develop and maintain referral relationships. Conduct community outreach and networking events. Follow up on all referrals promptly. Coordinate with the intake team to ensure timely admissions. Assist with social media and local marketing initiatives. Represent the company professionally in the community. Performance Expectations (First 90 Days) Make 15-20 in-person marketing visits per week. Build relationships with at least 50 referral sources. Generate 8-10 qualified referrals per month. Convert at least 2-5 referrals into active Medicaid clients each month.
