Better Days Home Health is a Virginia Department of Health licensed personal care agency serving patients across Hampton Roads, Virginia. We are looking for a Community Liaison to build and grow our referral network with hospital discharge planners, social workers, skilled nursing facilities, assisted living facilities, and physician offices throughout the region — including Sentara and Bon Secours systems. This is a commission-based contractor role built for someone who already has healthcare relationships or is coming from a clinical, social work, or medical sales background and wants to leverage those connections independently. No office hours, no micromanagement — you own your territory. What You Will Do

• Visit and build relationships with hospital discharge planners, social workers, skilled nursing facilities, assisted living facilities, and physician offices across Hampton Roads
• Educate referral sources on our services and intake process
• Follow up on referrals and keep the pipeline moving
• Log weekly activity through our referral tracking system How You Get Paid
• $500 per month ramp stipend for the first 90 days to cover gas and client meals while you build your territory — no strings attached
• $200 per admitted case — paid when a patient is admitted to our service
• $100 bonus per case that remains active at 90 days
• $50 per month for every active case beyond 90 days — ongoing as long as the case runs
• $600 per month retainer once you reach 5 or more admissions in a calendar month
• No cap on earnings What You Could Earn
• First 90 days (3 admissions per month): approximately $1,100 per month including the ramp stipend
• Building (5 admissions per month with several long-term cases): approximately $1,600 to $2,000 per month
• Established (8 to 10 admissions per month with a full long-term caseload): approximately $3,000 to $3,500 per month Who We Are Looking For

• Existing relationships with referral sources in Hampton Roads preferred but not required
• Self-motivated — this role is yours to build
• Reliable transportation — this role requires regular in-person visits across Hampton Roads
