In this vital role, you will serve as a key connector for young adults who are leaving the fostercare system and entering into their own apartment. The Supportive Services Resource Specialist will work with young adults who are seeking social, health, and community resources, ensuring they receive comprehensive support tailored to their unique needs. Your energetic approach and dedication will empower individuals and families to overcome challenges, access essential services, and improve their overall well-being. This position offers an exciting opportunity to make a meaningful impact in the lives of young people from diverse communities by providing expert guidance, crisis intervention, and resource navigation. Duties As part of a team, assist in tenant elibility screening. Identify FSET eligible young adults for referral for enrollment in FSET. Conduct thorough assessments of clients' needs through active listening and motivational interviewing techniques to develop personalized support plans. Identify and connect clients with appropriate social services, including Medicaid, Medicare, Section 8 housing, child welfare programs, and workforce development initiatives. Provide employment soft skills training, support groups, and group activities as requested by participants. Provide crisis intervention and crisis management support during urgent situations, ensuring safety and stability for clients experiencing emotional or behavioral health crises. Offer referrals and behavioral health resources to participants experiencing substance use and/or other mental health challenges. Assist clients with navigating complex systems such as social services, healthcare, and housing programs while maintaining clear documentation of case progress. Collaborate with multidisciplinary teams to supervise case management efforts . Facilitate culturally sensitive communication with diverse populations. Maintain up-to-date knowledge of social service policies such as Medicaid, Medicare, Child Welfare laws, and Section 8 regulations to ensure accurate resource provision. Maintain participant files and accurate data. Enter participant data, case notes, and tilling ifnormation in database in a timely manner. Complete and submit all required program reports for contracts on time. Bilingual or multi-lingual a plus with wage differential. Requirements A Bachelor's or Associate's Degree in a related human services field and at least one year of experience working with young adults, or an Associate's Degree in a related field and at least two years of work experience with young adults, or lived experience in the foster care system and at least two years of experience working in human services (paid or volunteer). Ability to work effectively with culturally diverse communities; bilingual skills are highly desirable. Understanding of challenges faced by young people aging out of foster care. Knowledge of community resources. Creativity and flexibility are esssential to work collaboratively with other community partners. Excellent writing skills for documentation and reporting; ability to communicate clearly across multiple platforms. Supervisory experience or mentoring skills are beneficial for team collaboration efforts. Familiarity with social services systems such as Child Welfare, Section 8 housing programs, workforce development initiatives, and childcare services.
