In-home and community-based Position Summary Bridging Gapss, LLC is seeking dependable, compassionate, and experienced CLTS Respite Care Providers interested in providing respite services to children and youth with disabilities in Waukesha County. This opportunity is ideal for individuals with experience in caregiving, disability services, behavioral health, special education, child development, social services, or other human-service settings who are passionate about supporting children and families. Respite services provide short-term relief to primary caregivers while ensuring children and youth receive safe, structured, supportive, and engaging care. Services may take place in the child's home, community, or other approved setting based on the child's authorized services and individual needs. About the CLTS Program The Children's Long-Term Support (CLTS) Waiver Program is a Wisconsin Medicaid program that provides services and supports to eligible children and youth with disabilities so they can remain in their homes and participate meaningfully in their families and communities. As a CLTS Respite Care Provider contracting with Bridging Gapss, LLC, you may provide individualized respite support to children and youth enrolled in the CLTS program. Services are based on each child's authorized needs and may include supervision, assistance with daily activities, behavioral and emotional support, recreational activities, and opportunities for community participation. Unlike a general respite or direct-support opportunity, this role specifically involves providing services to children and youth with disabilities through the CLTS program . Providers are expected to meet applicable service-plan, documentation, safety, confidentiality, and CLTS program requirements associated with the services they agree to provide. Current Service Opportunities Current service opportunities are primarily available Monday through Thursday . Individual service opportunities may range from approximately 5-10 hours , depending on authorized services and family needs. Independent contractors may indicate their availability and determine whether to accept offered service opportunities. The availability, frequency, location, and duration of opportunities may vary based on individual client needs and authorized services. Scope of Services Depending on the child's individual needs and authorized services, services may include: Providing safe, engaging, and developmentally appropriate respite support. Providing appropriate supervision and support consistent with the child's authorized service plan. Interacting with children and youth in a positive, respectful, and supportive manner. Assisting with activities of daily living when applicable, including feeding, dressing, personal care, or mobility support. Maintaining a calm, safe, and structured environment. Supporting children experiencing emotional or behavioral challenges using appropriate coping, redirection, and support strategies. Encouraging independence, choice, self-determination, and development of age-appropriate skills. Supporting recreational, social, and approved community-based activities. Promoting meaningful community participation and inclusion. Communicating professionally with parents, caregivers, and designated Bridging Gapss contacts regarding contracted services. Completing required service documentation, service-time records, and progress/service notes in accordance with applicable program and billing requirements. Using a computer or laptop to complete required electronic documentation. Maintaining confidentiality and appropriate professional boundaries. Reporting applicable incidents, concerns, or significant changes according to program and safety requirements. Qualifications CBRF or CNA certification required. At least one year of experience working with children, individuals with disabilities, or individuals experiencing emotional or behavioral challenges preferred. Experience in caregiving, child development, special education, behavioral health, social services, or other human-service settings is beneficial. Ability to independently provide services in family homes and community settings. Comfortable using a computer or laptop for electronic documentation. Strong communication, organization, and time-management skills. Ability to remain calm and use good judgment during challenging situations. Provider Requirements Providers must:
Be at least 18 years of age . Successfully complete applicable caregiver/background screening. Meet applicable CLTS provider, credentialing, and program requirements. Complete applicable orientation, training, or compliance requirements associated with contracted services. Have reliable transportation for applicable service opportunities. Possess a valid driver's license when driving is applicable to contracted services. Maintain applicable automobile insurance when using a personal vehicle in connection with contracted services. Be able to travel to accepted service opportunities within Waukesha County. Physical Considerations Physical requirements may vary based on the needs of the child and the service opportunity. Some opportunities may involve standing, walking, bending, participating in activities with children, providing mobility assistance, or lifting/assisting with up to approximately 30 pounds .
