Full Job Description Duties and Responsibilities:

The In-Home Outreach Counselor assumes the following duties and responsibilities:  Provide in-home outreach counseling to provide support and educate families on parenting skills, positive discipline, laws regarding child abuse and neglect, goal setting, and family organization;  Provide supplemental services: child focused activities; child follow-up visits; parenting training; teaching and demonstrating homemaking skills; counseling on substance abuse, domestic violence or anger management, and career need issues; substitute adult role model; transportation; and emergency housing;  Develop and utilize community resources to provide linkage services for drug testing, substance abuse assessment and treatment services, etc.  Complete intakes and assessment forms and maintain up-to-date FAF primary web-based Electronic Case Records system and hard copies case records and back up documents on each client and family served, and other reports in a timely manner;  Be available to provide emergency services as needed;  Attend MCPC and all other meetings and trainings required by the program;  Strictly adhere to confidentiality, privacy and Cyber security policy and protocol;  Performed other related duties as assigned
