REQUIREMENTS
A Master's Degree from an accredited college or university, specializing in psychology, social work, counseling, or other related human services field Current, appropriate licensure/certification issued by the State of Michigan Possess a CAADC, CADC or file for a development plan within 30 days of employment One year providing direct treatment services to adults in a behavioral health inpatient or outpatient setting Demonstrate knowledge of diagnostics, psychopharmacology, and supportive treatment approaches as applied to a substance abusing adult population
RESPONSIBILITIES
(summary): The SUD Therapist will also work with drug/sobriety court clients. Intake Facilitate new patient orientation group, as assigned, and provide accurate information regarding clinic and corporate CMH services. Complete assigned new intake comprehensive bio-psychosocial assessment. Formulate preliminary DSM V Diagnosis. Identify issues that may exist regarding safety (personal and environmental), natural support, and community involvement. Identify referrals needed for comprehensive treatment, including medical, nutrition, substance abuse, STD/HIV, education, and vocational training. Identify Targeted Case Management needs for CMH-eligible patients and complete referral recommendation. Obtain releases of information for coordination of care. Demonstrate knowledge of patient payor source. Treatment Planning and Progress Reviews Complete an individualized Treatment Plan for each patient. Utilize the patient's Person-Centered Questionnaire in development of treatment plan and support person participation. Complete Relapse Prevention Plan within 30 days of admission. Treatment Plan problem areas, goals, and objectives address the patient's DSM five-axis diagnosis and areas identified on the bio-psychosocial assessment. Demonstrate use of natural supports in Treatment Plan. Complete Treatment Plan Reviews. Update Treatment Plan via a TPR following a change in clinical status Discharge Process Document on Treatment Plan criteria and anticipated date for planned discharge. Update changed discharge criteria on the TPR. Discharge patients with no contact for 45 days. Provide written notification to Medicaid recipients of intent to terminate services. Complete Discharge Summary within 21 days of discharge date. Complete a Continuing Care Plan for each patient discharge at the last session for planned discharges and within seven days for unplanned discharges. Ongoing Provide individual supportive, solution-focused psychotherapy not less than once every 30 days for each patient on caseload. Provide group therapy, as assigned, demonstrating utilization of supportive and/or psycho-educational approach. Start and stop therapy, collateral, and consultation sessions as scheduled, unless crisis intervention services require otherwise. Initiate contact, by phone or in writing, to non-compliant patients within 24 hours of a missed appointment. Report medication issues, including non-compliance and adverse side effects, to clinic psychiatrist and/or CSS or Program Manager immediately. Monitor substance use via Urine Drug Screens, as indicated. Pay is based upon years of related experience . Full license is paid at a higher rate.
