Collaborate with healthcare teams to ensure holistic care and coordinate services. Maintain accurate medical documentation using

ICD-10/ICD-9

coding standards. Monitor progress and evaluate treatment effectiveness, offering case management support. Qualifications include valid licensure (LCAS, LCSW, or LCMHC), experience with substance use, and proficiency in assessments and therapy techniques. Strong communication, organizational skills, and ability to work in a multidisciplinary team required. Willingness to train individuals with provisional licenses is preferred.
