In this vital role, you will provide compassionate, evidence-based counseling to clients struggling with substance use disorders and co-occurring mental health conditions. Your dedication will help foster resilience, promote healing, and support positive behavioral change within diverse community settings. Responsibilities Conduct comprehensive patient assessments utilizing DSM-5 criteria and medical terminology to determine substance use severity and co-occurring disorders. Deliver individual and group counseling sessions employing cognitive behavioral therapy, motivational interviewing, and behavioral therapy techniques to facilitate recovery. Develop personalized treatment plans aligned with clinical documentation standards, ensuring compliance with HIPAA regulations and medical records management. Perform crisis intervention and crisis management to support clients experiencing acute mental health or substance-related emergencies. Collaborate with multidisciplinary teams to coordinate case management, home & community care, and hospital-based services for clients with complex needs. Maintain accurate electronic health records (EHR/EHR) systems, documenting patient care, intake procedures, progress notes, and treatment outcomes in accordance with clinical standards. Engage in ongoing research and data collection efforts to enhance evidence-based practices within behavioral health and addiction medicine. Requirements Valid certification as an Alcohol and Drug Counselor (CADC or equivalent) with active licensure where applicable. Proven experience working with individuals with substance use disorders, co-occurring mental health conditions, and PTSD care needs. Strong knowledge of psychotherapeutic modalities including motivational interviewing, cognitive behavioral therapy, applied behavior analysis, and behavior management strategies. Familiarity with medical documentation standards such as
ICD-9/ICD-10
coding, CPT coding, DSM-5 diagnostic evaluation, and ICD coding systems. Experience utilizing electronic health records (EHR/EHR) systems for patient assessment, treatment planning, and clinical documentation. Excellent skills in crisis management, social work practices, case management, and working within HIPAA guidelines to ensure patient confidentiality. Ability to work effectively in home & community care environments as well as hospital settings; experience with medical terminology is preferred.
