Job Description

Receive 17% Weekday Nights, 26% Weekend Nights and 15% Weekend Day shift differentials Minimum Offer $24.29/hr. Maximum Offer $ 36.43/hr. Compensation Disclaimer Compensation for this role is based on a number of factors, including but not limited to experience, education, and other business and organizational considerations.

Full Time Shift:
Days Position Summary:
Counsel and aid individuals and families requiring assistance dealing with substance abuse problems, such as alcohol or drug abuse. Refer client to other support services as needed. Monitor condition of client to evaluate success of therapy and adapt treatment program as needed. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include Neonate, Pediatric, Adolescent, Adult and Geriatric age groups.
Detailed responsibilities:
INFORMATION
  • Obtain information about patients through interview, case history, and observational techniques; evaluate data to identify causes of problems and to determine advisability of counseling or referral to other specialists
PATIENT CARE
  • Formulate program for treatment and rehabilitation of client, using knowledge of alcohol and drug abuse problems and counseling and treatment techniques
INTERVENTION
  • Provide and/or arranges for therapeutic interventions as appropriate for patients or clients in a crisis condition and for those with serious disturbance problems COUNSEL
  • Counsel family members to assist family in dealing with providing support for client
PATIENT CARE
  • Follow up to determine reliability and validity of treatment used; make recommendations regarding modification to services and service delivery COUNSEL
  • Refer client to other support services as needed, such as medical evaluation and treatment, social services, and employment services
CONSULTATION
  • Consult with other legal and treatment agencies and individuals in relation to patient/client records, rights, and responsibilities REPORT
  • Prepare and maintain reports and case histories
POLICIES
  • Participate in the review of clinical issues and program policies and procedures COURT
  • Prepare documents for presentation in court and accompany client to court as needed MEET
  • Attend staff meetings and team conferences
KNOWLEDGE
  • Maintain current knowledge base on counseling issues regarding substance abuse through conferences, educational requirements for licensure, and applicable current literature

  • Follow patient safety-related policies, procedures and protocols

  • Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes

  • Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk

  • Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
PATIENT SAFETY 5
  • Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"
