ORGANIZATION
Weston United Community Renewal Inc. is a not for profit organization located in Harlem, New York. For over 30 years, Weston United has provided housing and supportive services to participants with histories of chronic homelessness, mental illness, and substance abuse. Weston United employs over 100 employees who contribute to a rich and diverse work environment.
Essential Position Functions:
Conduct client intake, assessment, and screening processes while documenting all interactions in client databases, and develop individualized service plans based on consumer strengths and needs. Provide crisis intervention, facilitate group sessions, and connect clients with community resources for support. Ensure timely and accurate documentation of client contacts, adhering to agency policies and regulatory guidelines, and collaborate with social service staff to identify high-risk clients and facilitate referrals to outpatient and inpatient services; and any other duties as may be assigned.
Educational Requirements:
This position requires a Bachelor's degree in Social Work or related field and at least two years of related work experience in mental health and/or substance abuse assessment and treatment.

Credential required.
Qualifications and Experience :
The candidate must have at least two years of experience working with adults who have a severe and persistent mental illness with a history of substance abuse. Candidate must have excellent time management skills; stress resistant, flexible; team player; open to feedback; creative; excellent organizational skills; willingness to perform CPR/First Aid/AED/CPI/Naloxone. Proficient in Microsoft Office applications including Word, and Excel and knowledge of DHS CARES and AWARDS preferred. This position requires some evening hours.
