Position Summary The Assertive Community Treatment (ACT) Clinician provides community-based, recovery-oriented services to adults living with serious mental illness and co-occurring disorders. The clinician is a key member of the ACT interdisciplinary team and delivers treatment and support in both clinical and natural settings, meeting clients where they are — whether at home, in the community, or other community environments. This position emphasizes individualized care, engagement, and rehabilitation that promotes recovery, autonomy, and community integration. Responsibilities Provide community-based clinical services, including individual counseling, crisis intervention, skill-building, and supportive therapy. Participate as a core member of the ACT interdisciplinary team, collaborating daily with psychiatrists, nurses, case managers, peer support specialists, and vocational specialists. Conduct ongoing assessments of client needs, strengths, and progress to inform individualized treatment and recovery planning. Develop and maintain individualized service plans focusing on symptom management, community living skills, relapse prevention, and social functioning. Engage clients proactively to promote participation in treatment, often through assertive outreach and field-based work. Provide crisis intervention and on-call support as part of the ACT team's rotating schedule. Coordinate care across systems, including primary care, housing, employment services, and community organizations. Facilitate team-based communication by contributing to daily team meetings and case consultations. Document all client interactions in accordance with agency and regulatory standards. Participate in supervision, training, and continuing education to maintain professional competency and support the integrated, evidence-based ACT model. Qualifications Master's degree (LPCC, LCSW, LMSW, LMHC) or Bachelor's degree with licensure as LSAA, LADAC, or other applicable certification. Licensed or license-eligible in the State of New Mexico as a behavioral health professional. Experience working with individuals with serious mental illness, co-occurring disorders, or in community-based services preferred. Strong interpersonal skills and ability to work collaboratively within a multidisciplinary team. Comfortable providing services in community settings, including clients' homes or other non-office environments. Valid driver's license, reliable transportation, and ability to travel within the service area. Preferred Qualifications Previous experience on an ACT or mobile crisis team. Training in evidence-based interventions (e.g., Motivational Interviewing, DBT, CBT). Bilingual in English/Spanish. Familiarity with harm reduction, and trauma-informed care.

Pay:

$60,000.00 - $80,000.00 per year
