Residential Caregiver The Collier Group Home, located in Redbank, NJ is seeking a Full Time Case Manager. Encourages youth to seek balance in all aspects of life, such as relationships, interactions, and independence. The therapeutic relationship encourages them to move forward from past experiences that may negatively affect their well-being. Incorporates the Sanctuary Model philosophy, Nurtured Heart Approach and other crisis intervention modalities into daily interactions with the youth, community members and staff. Duties/Responsibilities 1. Provide support counseling to the youth and family served, incorporating the Sanctuary Model and Nurtured Heart principles. 2. On-board and orient new employees to Collier Group Home and make sure all team members are completing all necessary training. 3. Provide guidance and counseling for all youth in the following areas: adjustment to Group Home living, open new avenues and/or strengthen ties with family, academic life, vocational planning, discharge plans, as well as keeping in contact with outside therapist. 4. Prepare all meetings and work with the youth to develop their Individualized Plan. 5. Complete biweekly goal reviews for each youth and communicate these reviews to the treatment team via the electronic log. 6. Prepare all files when a new youth is admitted and complete a youth orientation. 7. Meet biweekly with the consulting psychiatrist regarding monthly medication review and communicate needs to youths' guardian and team. 8. Meet weekly for supervision and discuss concerns and areas of growth. 9. Communicate with Collier High School administration, social worker and director of social services as needed. 10. Conduct family sessions when appropriate and offer NHA training sessions to parents and guardians. 11. Be available for crisis intervention. 12. Maintain contact with past youth and assist when appropriate. 13. Perform other duties as assigned. 14. Maintain agency adherence to confidentiality and HIPAA requirements. 15. Complete other projects or tasks as assigned by management. Education/Experience 1.Master's Degree in Social Work or related field (mental health counseling) 2. Unlicensed or seeking supervision to become licensed, with the organization providing appropriate supervision, support, and professional development opportunities to assist employees in working toward and obtaining their required licensure. 2. Acquire and obtain required professional licenses. 3. Valid NJ driver's license with clean driving record. 4. Satisfactorily pass Criminal History, Fingerprints, TB Screening, physical, CARI and/or Sexual Misconduct and Child Abuse Disclosure. 5.
