Job Description

Program Overview Sarah's House is a structured residential program serving up to six female youth ages 15-17 who require stability, trauma-informed support, and preparation for adulthood. The program uses a phase-based developmental model emphasizing permanency, life skills development, emotional healing, and relationship-based care. Youth typically remain for 12-24 months depending on their permanency plan and individual progress. The program promotes safety, healing, and independence through: Trauma-informed care Consistent, structured routines Relationship-based mentorship Skill-building and accountability Permanency-focused planning ____________________________________________________________________________ Position Summary The Sarah's House Permanency Specialist provides direct supervision, crisis intervention, mentorship, and case support for youth in residence. Staff contribute to a safe, supportive, trauma-informed environment where youth can build independence, emotional regulation, and essential life skills. This role involves daily supervision, life-skills facilitation, crisis response, documentation, and supportive case management consistent with Sarah's House's structured model of care. Key Responsibilities Permanency Planning & Case Management Develop, implement, and monitor individualized permanency plans for each youth Collaborate with case workers, therapists, families, and external stakeholders Participate in court reviews, staffing meetings, and case planning sessions Maintain accurate, timely documentation in compliance with agency and state requirements ________________________________________________________________________________ Youth Development & Life Skills Provide guidance in independent living skills (e.g., budgeting, hygiene, cooking, job readiness) Support educational goals, including school attendance, tutoring, and graduation planning Assist youth in securing employment, internships, or vocational opportunities Facilitate life skills groups and workshops ________________________________________________________________________________ Crisis Intervention Provide empathetic, strengths-based crisis response to youth. Use coaching, redirection, and supportive interventions instead of punitive approaches. Complete special incident reports accurately and promptly. Offer emergency shift coverage when required. _________________________________________________________________________ Life Skills & Program Facilitation Facilitate life-skills programming, including: Financial literacy Cooking, cleaning, home management Emotional regulation and communication Career readiness and vocational skills Support youth progress through the four Sarah's House phases (Foundations, Growth, Independence, and Transition). Family & Permanency Facilitate family engagement and visitation when appropriate Support reunification efforts or alternative permanency options (e.g., guardianship, adoption) Help youth build and maintain lifelong connections with supportive adults _______________________________________________________________________ Case Management Support Assist with intake, assessments, documentation, and maintenance of resident files. Support development and tracking of individualized permanency plans (reunification, kinship, guardianship, extended foster care, etc.). Participate in Child & Family Team meetings, permanency planning sessions, and family engagement activities. Complete ongoing case management for each youth, ensuring all progress notes, case files, service plans, and goal updates remain accurate and current. Assist with data, updates, and narrative summaries required to build monthly program reports for leadership and licensing requirements. ____________________________________________________________________________ Group & Family Activities Facilitate groups including life skills, harm reduction, goal setting, and restorative discussions. Participate in family counseling sessions, visitation, and ongoing family involvement when appropriate. ______________________________________________________________________________ Professional Development & Training Attend all required training: TCI, trauma‑informed care, program policies, and documentation standards. Participate in team meetings, case reviews, and quality improvement initiatives. _____________________________________________________________________________ Daily Program Operations

Minimum Qualifications Bachelor's degree in social work, Psychology, Human Services, or related field At least 1 year experience working with vulnerable youth or youth in crisis. Ability to manage emotional and behavioral challenges using trauma-informed practices. Strong communication, organization, and teamwork abilities. Ability to multitask and maintain flexibility in a dynamic residential setting. Availability for emergency coverage when needed. Must pass all required background checks, including a clean 3-year driving record. Valid driver's license is required. _________________________________________________________________________________ Work Schedule & Conditions Various 12-hour shifts scheduled monthly by the Program Coordinator The program is open 24hrs/365 days a year Evening, weekend, and holiday shifts required Residential home environment with evolving program needs __________________________________________________________________________________
Compensation & Benefits Hourly Rate:
$18.00 (non‑exempt) Professional development and training opportunities
Paid Time Off Pay:
$18.00 per hour Expected hours: No more than 40.0 per week
