Job Description

The Youth Crisis Outreach Team (YCOT) program is a crisis stabilizing resource specially designed to support youth and families in crisis to prevent escalation to more acute settings when resolution in home, school or community setting is more appropriate. YCOT is a specialized resource embedded within the crisis service continuum, working alongside MCOT to collectively provide support 24 hours a day, seven days a week. YCOT will provide developmentally appropriate, in person response with specially trained staff. YCOT will used Trauma informed interventions and strategies to de-escalate a youth crisis, aid in relapse prevention and safety planning, and will offer support to the youth and their family, or other primary caregiver, for a minimum of 30 days and up to 90 days after crisis. The
YCOT LPHA
conducts intake and diagnostic assessments, administers CBT counseling by conducting individual and/or family sessions, case-management, and psychoeducation in accordance with Texas Resilience and Recovery Guidelines. Work performed will occur in community settings as necessary to meet client needs, in addition to some office-based services. Will be authorizing Levels of Care based on Uniform Assessment (UA) recommendations. Develops individualized person recovery centered plans with measurable outcomes, reviews and updates plan every 90 days, and participates in clinical consultations. Works under moderate supervision with latitude for the use of initiative and independent judgment.
