We are currently seeking a skilled and compassionate Speech-Language Pathologist (SLP) to join our San Ramon-based team. In this role, you will evaluate and treat patients experiencing speech, language, cognitive, and swallowing disorders , helping them regain communication skills and improve overall quality of life in the comfort of their homes. Key Responsibilities

• Perform comprehensive speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans based on patient needs and clinical goals
• Provide therapy interventions to improve articulation, fluency, cognitive-linguistic abilities, and swallowing safety
• Educate patients and caregivers on communication strategies, swallowing safety, and home exercises
• Collaborate closely with PTs, OTs, RNs, and caregivers as part of an interdisciplinary care team
• Monitor patient progress and modify treatment plans as needed
• Maintain accurate documentation in EMR (WellSky) in compliance with Medicare and agency standards
• Communicate treatment updates and outcomes to physicians and care teams Qualifications
• Active, unrestricted California Speech-Language Pathology license
• Master's Degree in Speech-Language Pathology from an accredited program
• Certificate of Clinical Competence (CCC-SLP) preferred
• Minimum 2 years of clinical experience (home health or adult rehabilitation preferred)
• Current CPR certification
• Strong communication, documentation, and patient education skills
• Experience with EMR systems (WellSky or similar) preferred
• Valid California driver's license and reliable transportation Benefits
