Spiritual Care Counselors at DHHC are integral members of a multi-disciplinary health care team that are responsible for the provision of spiritual care services to patients and families/caregivers of the Agency, either directly or through coordination of care with other spiritual counselors.

The Spiritual Care Counselor will:

Assure spiritual assessment of patients and families in the hospice program and appropriate services are rendered in a timely manner to meet regulatory guidelines. Provide direct spiritual care to patients and families/caregivers. Serve as liaison to community chaplains and spiritual counselors. Document direct services and ongoing communication with community resources. Provide funeral or memorial services for patients as requested. Provide bereavement support to families and caregivers for up to a year after the death of the patient. Perform hospice informationals for patients and families Qualifications Graduate of Theological Studies from accredited university required. Experience with database software or electronic medical record software required. At least 1 unit of Clinical Pastoral Education required. Completion of Clinical Pastoral Education residency preferred. Two years of experience in health-related field preferred. Hospice experience preferred. Required Licensure/Certifications None
