Staff Registered Nurse

The registered professional nurse in the adult medical day care setting shall be responsible i for, but not limited to the following: 1. Interviewing the participant and caregivers in order to evaluate the participant's health status and healthcare needs; 2. Maintaining the standards of nursing practice including, but not imited to: i.

Monitoring of identified medical conditions, ii.

Administration and supervision of prescribed medications and treatments, iii. Coordinate rehabilitative services, iv.

Developing of a restorative nursing plan, v.

Monitoringof clinical behavior and nutritional status, vi.

Assisting with the maintenance or redevelopment of the activities of daily living skills, viii.

Implementing infection control procedures and ix.

Communicating findings to the attending physician 3.Managing medical emergencies 4.Documenting the nursing services provided, including the initial assessment and evaluation of the participant's health care needs quarterly. 5.Development of the nursing component of the individualized plan of care, evaluation of the participant's progress in reaching established goals and defining the effectiveness of the nursing component of the individualized plan of care. 6.Overseeing the development of the initial individualized interdisciplinary plan of care; 7.- Alerting others involved with the participant's care about changes in status and th need to change the individualized interdisciplinary plan of care; 8.Developing community medical referral resources and maitaining ongoing communication with those providers; 9.Linking the participant to necessary heath care services outside the program; 10.Coordinating the services provided by other staff to meet the mutually identified healthcare and psychosocial needs of each participant; 11.Provide in-service training to facility staff about the participant's healthcare needs. 12.Providing health education for a participant's family or primary caregiver; and 13.Serving as an advocate to assist the participant/caregiver to resolve problems. 14.Follow -up on pharmacist's consultant recommendations. 15.Assistance and supervision to participants with Activities of Daily Living and monitoring of vital signs. 16.Responsible for prior authorization request to the participant medical insurance, admission and discharges of participants to and from the program. 17. Maintain an accurate participant's master list including discharges. 18.Coordination of nursing services with the Interdisciplinary Team. The facility nursing staff shall assure that nursing services provided to participants are coordinated with health services currently received at home, as participants in time of need.

Pay:

$40.00 - $42.00 per hour Expected hours: 35 - 40 per week
