Job Description

SUMMARY:

ESSENTIAL FUNCTIONS
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Assess patients with substance abuse concerns using NIDA compliant tools. Identify risk factors of substance use, withdrawal, intoxication, overdose, and relapse. Utilize relapse prevention efforts such as psychoeducation, coping skills development, and community referrals. Develop individualized strategies to help patients manage triggers and trauma history. Provide substance use screening and counseling to patients referred by facility staff, medical staff, or as identified by the courts. Refer patients who may benefit from behavioral health intervention to onsite qualified mental health professionals. Collaborate with jail and medical staff on the management and treatment of patients with identified risk of withdrawal, as well as those eligible for medication assisted treatment (MAT) services. Coordinate with county behavioral health services for continuity of care, prevention efforts, and discharge planning via sharing of pertinent patient information. Complete documentation of assessment and treatment and maintain records for referral tracking and continuous quality improvement (CQI). Facilitate substance use groups as requested. Maintain the integrity and confidentiality of organizational and patient information. Demonstrate commitment and dedication to providing substance abuse support and provide training designed to solve problems and make lives easier. Partner with behavioral health, medical, and management teams to facilitate teamwork to collaboratively accomplish organizational goals. Perform high-quality work that meets organizational standards and demonstrates a higher standard of patient care. Maintain accurate and up-to-date medical records and reports. Serve as the liaison between service providers, to include both internal and external personnel. Perform all other duties as assigned.

REQUIRED EDUCATION AND EXPERIENCE
High School diploma required. 2+ years' experience counseling those with substance abuse challenges OR an equivalent combination of education and experience required. Knowledge and training in substance abuse and related health issues and recovery protocols (including 12 step programs) required. CPR certification must be obtained prior to hire and maintained while in this role. Hands-on or online certifications are accepted. Associate or bachelor's degree in related field preferred. Knowledge of the local treatment community and referrals preferred. Experience in individual and group counseling preferred. Experience with medication assisted treatment (MAT) preferred. Must possess a valid driver's license and be able to operate a motor vehicle.
