Job Description
Team Leader, Disability Services Summary Under the direct supervision of the Program Director of Disability Services, the Team Leader assist the Program Supervisor in managing all aspects of the program Position Type Full Time, Non-Exempt Essential Functions Attend Residential meetings (IPP meetings, quarterly meetings, HRC meetings, and staff meetings, etc.) Monitor all documentation weekly (MARS, contact, recreation, grocery receipts, Leave reports, and WEX receipts, etc.) Follow up with the home ensuring that all medication inventories are completed; necessary medications are re-ordered; meds are taken as prescribed and documented correctly Review all PO's ensuring that all medications prescribed are logged correctly Follow all procedures as it relates to EZ Labor (clocking in & out, training new staff to clock in & out, reviewing time cards, submitting edit sheets, and payroll adjustments) Provide back-up/weekend coverage as needed Assist Supervisors with paper work as assigned and program errands. Responsible for care of residents unable to report to work Schedule and arrange residential doctor's appointments. Review of active treatment and daily shift communications Report critical incidents/significant concerns to supervisor Maintain training requirements Provide 24 hour on-call supports with weekly rotated on-call shifts. Assist supervisor with maintaining state requirements Projects and duties as assigned Facilitate staff meetings as needed During an emergency or inclement weather, you must coordinate staffing services for individuals served. (Evacuate/Relieve staff upon return from evacuation/shelter in place) Evacuate with Community Living Services consumers Projects duties as assigned Maintain home and monitor health and safety requirements in accordance with state and federal regulations Attend IPP meetings Assist Program Director and with maintaining CARF/State requirements Assist Program Director with implementing QA/QI protocol Assist clients to attend doctor's appointments, as needed Provide staff scheduling in group homes Assist with Supervision of Direct Service Workers and program functions CMA certification (once requirements are met) Prepare grocery list and complete grocery shopping as needed Attend and participate in pertinent meetings Maintain required documentation (shift logs, MAR, home visit form, incident, reports, requests for money, etc.) Empower individuals to develop leisure, social and employment skills Assist program individuals with implantation of service plans Assist and train individuals in performance of activities of daily living Transport individuals to and from destination Competencies 1.) Team-building Supervisory Responsibility This position directly assists to supervise residential staff and individuals served. Required Education and Experience 1. High School diploma 2. Minimum 1 years' experience working with individuals with developmental disabilities 3. Supervision and Leadership 4. Organization and Communication Skills 5. Valid driver's license Physical Demands While performing the duties of this job, the employee is frequently required to stand; walk; sit, use hands, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
