Job Description

Performs data tracking duties and track PCMH data in conjunction with the health care team.
Duties and Responsibilities include:
Weekly reviews telephone encounter unassigned documents to ensure completed/review. Flu shot registry - generates appropriate patient reminder calls. Uses registry to report weekly flu surveillance reports to public health. Assist Office and Nursing staff with coordination of care Works with the Providers and care management team to ensure results are reviewed Collects data for specific projects as they develop Other duties as required
Experience:
one year work experience in an outpatient primary care environment with knowledge of electronic medical records, medical terminology
Pay:
$18.00 - $27.23 per hour
