The primary responsibility of the person in this position is to provide services for the Medicaid Alternative Pathways to Success (MAPs) program. The MAPs program involves working with people with intellectual and developmental disabilities to become independent in the home, community, and workplace through Enabling Technology supports.

DUTIES AND RESPONSIBILITIES
Virtual Community Resource Map (VCRM) Produce a person-centered digital map (similar to or Google Maps) that identifies resources and places of interest to serve as a Person-Centered Plan. Developing the VCRM includes the following: 1. Working with individuals, their families, and other people in their support network the aim of helping the person to identify and make connections with their chosen activities, resources, and interests. 2. Exploring opportunities in the community and learning how to become involved and integrated with more people, groups, and activities. 3. Forming and fostering relationships with people who are not paid supports. 4. Determining interests in further education and connecting the person to resources for education. 5. Ensuring all details of points of interests are included in the VCRM. 6. Assisting the person and their support network in using and updating the VCRM. Enabling Technology Collaborate with Innovation Coordinators, Enabling Technology Champions, and the STAR Center Assistive Technology team to recommend innovative low-tech and high-tech solutions for people with intellectual and developmental disabilities. Enabling Technology is built into every aspect of MAPs. Independence Coaching Support a person with an intellectual disability to use enabling technologies to achieve independence milestones. All milestones are person-centered and therefore will have individualized criteria. 1.
Home Safety:
Demonstrates the skills necessary to spend time alone at home. Includes the ability to cook safely, recognize and respond to emergencies, and showing discretion with visitors. 2.
Personal Hygiene and Self-Care:
Demonstrates the skills necessary to plan and prepare for meals, maintain personal hygiene and appearance, and dress themselves appropriately. 3.
Health Management:
Demonstrates the ability to manage health, including taking medications and knowing how and when to see a doctor. 4.
Financial Management:
Demonstratestheability to maintain personal finances, including having a personal checking and savings account, maintaining a line of credit, paying bills on time, budgeting, and understanding how to avoid financial exploitation. Community Navigation Support a person to use enabling technologies to achieve the following person-centered milestones: 1.
Independent Travel:
Independently and consistently initiates transportation to preferred in the community, through use of the VCRM or spontaneously. 2.
Community Relationships:
Has established recurring and natural relationships with people who are not paid supports. Shows proficiency in navigating and maintaining social relationships. 3.
Community Activities:
Leverages information or relationships and develops new interests and opportunities. Employment Innovation Coordinate with the STAR Center employment team to support a person in achieving the following milestones: 1.
Pre-Employment:
Has made informed decisions to identify employment, volunteering, or internship interests with the skill sets necessary. Includes meaningful activities to learn about strengths, skills and interests and receiving benefits counseling. 2.
Employment:
Person obtains competitive, integrated employment and works for 30 days. Includes learning how to use reliable transportation and working with a job coach. 3.
Career Development:
Person has advanced in their job through greater responsibilities, promotions, or has enrolled in a post-secondary program. 4.
Independence:
Person is successful in their job and no longer needs in-person supports. Administrative Activities 1. Maintain confidential records on each person in the MAPs program. 2. Ensure all notes for persons supported are entered in by the end of the week each week. 3. Ensure all monthly reports are submitted by the 5th day of the following month. 4. Ensure all initial reports (such as assessments) are submitted within 10 business days of occurrence. Preferably within 1-2 business days. Additional Activities 1. Commit to person-centered, technology first, and employment first principles and practices. 2. Follow all policies and procedures from contracts, grants, and the STAR Center as a whole. 3. Comply with all applicable federal, state and local rules and statutes. 4. Provide orientation and awareness about the MAPs in the community. 5. Participate in training programs regarding Enabling Technology and MAPs. 6. Perform other duties as may be assigned.
KNOWLEDGE AND REQUIRED TRAINING
At a minimum, the candidate must: 1. Be 21 years or older and able to effectively read, write and communicate verbally in English. 2. Be able to read and understand instructions, perform record-keeping and write reports. 3. Pass criminal background checks per Tenn. Comp. R. & Regs. 1240-08-03-.05 4. Not be listed in TNDOH Abuse Registry or TN Sexual Offender Registry. 5. Have an associate's degree or bachelor's degree from an accredited college or university. 6. Have two (2) years of successful work experience in providing supports for people with intellectual and developmental disabilities. Candidates applying for this position should possess: 1. Proficiency with general computer usage, web applications, and office suites. 2. Proficiency with smartphones and mobile technologies. 3. A strong desire to support and work with people with intellectual and developmental disabilities. 4. A passion for technology and its use in supporting people with disabilities. 5. A strong sense of professionalism and ability to be a role model to persons supported. Strongly preferred candidates will possess one or more of the following: 1. A degree and background in special education, occupational therapy, social work, or similar fields. 2. Experience with person-centered thinking, planning, planning, practices, and tools. 3. Experience with enabling technology, assistive technology, and remote supports. 4. Expertise with smart home technologies, both low-tech and high-tech. Post-Hire Trainings 1. All training required for DDA Direct Support Professionals. 2. Enabling Technology training courses in Relias. 3. Virtual Community Resource Mapping training. 4. Work toward a Tech First SHIFT Enabling Technology Integration Specialist Certification (ETIS). 5. Work toward an ACRE Basic Certificate of Achievement in Employment Services and complete all trainings to become a DIDD Job Developer.
PERSONAL RELATIONSHIPS
The person selected for this position must establish and maintain an effective liaison with individuals both within and outside the Center. This person must have an empathetic understanding of people with intellectual and developmental disabilities and able to relate to them in a positive and supportive fashion. The person will also be required to build and maintain positive relationships with outside agencies and with other departments at the STAR Center.

TRAVEL REQUIREMENTS
This position requires frequent traveling between home and community locations within Jackson and the greater West Tennessee area. It may also involve transporting persons supported to visit points of interest. A valid driver's license, a reliable personal vehicle, and a good driving record with an appropriate level of automobile insurance is expected; however, if applicant is unable to drive for any reason, reliable personal transportation is a requirement of this position.
