Job Description

Job description: The Social Care Network Specialist conducts screenings to assess a community members' needs and eligibility for the Social Care Network and/or Care Management programs. The primary goal of the Social Care Network Specialist is to ensure individuals who have physical, behavioral or social health needs get access to the care and services they need to be healthy.
Essential Functions:
Manage incoming referrals for screening and navigation to ensure successful and timely connections for community members Conduct Screening through the Social Care Network (SCN) Determine if the client is appropriate for the SCN Navigation Services or Health Home Care Management (HH CM) services or both. If eligible for the SCN, complete the Eligibility Assessment If eligible for HH CM, aid in the Outreach process while coordinating with the HH CM Team. Link clients to appropriate community resources when applicable. Collaborate with community service providers to gain a better understanding of local resources. Maintain effective communication with internal team members, community members, and partner organizations to ensure acceptance, resolution, or redirection of referral requests. Attend community events and client appointments when needed. Ensure client follow through prior to warm handoff. Ensure compliance to all government and agency regulations/policies. Adhere to required program and internal deadlines. Perform all other duties relevant to the position as requested.
Position Requirements:
Knowledge and understanding of health equity, social drivers of health, and social care data Ability to build collaborative working relationships with others inside and outside the organization Demonstrate ability to use various technology platforms to ensure successful and timely referral connections are made. From $40,000.00 per year
