Job Description The Social Care Network Specialist will provide focused outreach and services to Medicaid eligible neighbors across Syracuse through the Social Care Network. The Specialist will complete screenings and assessments with neighbors and provide Enhanced Care Management services that support neighbors in overcoming barriers to health, wellbeing, and opportunity.
